How does admission work?

First you visit and we talk about what your parent needs. Then your parent's doctor fills out Florida's Form 1823, the health assessment the state requires, and you sign our admission agreement.

1. Call (786) 998-9736 to set up a visit.
2. Ask your parent's physician, APRN or physician assistant to complete Form 1823. We can send it to the office.
3. Read and sign the admission agreement. We go through it with you, and it includes Florida's information on advance directives and the Do Not Resuscitate Order form.
4. Ask the doctor to send all prescriptions, including over-the-counter medicines, before move-in.
5. On move-in day, bring a photo ID, insurance cards, current medications, and any health care surrogate, power of attorney or living will papers you have.
6. First and last month's rent are due at move-in.
